We know that your time is valuable, so we see patients by appointment. We ask you to call in advance so we can reserve a time for you. We do not require a doctor's referral to see you for a urological problem. If you need to cancel your appointment, please notify us as far in advance as possible.

What to Bring to the Appointment

• Your insurance card
• Physician referral forms if required by insurance
• A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
• Pertinent information about your medical and surgical history
• Any recent x-rays or appropriate records you may have

Fees and Payments

We make every effort to hold down your costs for medical care. To help us manage these costs, we do ask you to pay for your office visit at the time of service. If you will need surgery, our office manager will be happy to work with you to make payment arrangements.

Insurance and Insurance Forms

We work hard to simplify the preparation of insurance claims. This lets us reduce costs to our patients that are unrelated to the delivery of good medical care. Some companies pay a fixed amount for certain procedures, while others pay a percentage of the charge. Please remember that insurance is a method of reimbursing the patient for fees paid to the doctor, not a substitute for payment. It is your responsibility to pay any balance not paid by your insurance company.

Telephone Calls

We encourage our patients to call with any medical questions or problems. Our staff, who have experience in urological health issues, will take your message and relay it to the doctor. Your call will be returned at the earliest opportunity. Please alert our staff if you are calling about an emergency situation, so we can give your problem our immediate attention.

After Hours Telephone Calls

If you must contact the doctor after hours, your call will be forwarded by our answering service to the doctor. Your call will be returned at the earliest opportunity. Call our office number: 541.889.7205 or toll-free 877.374.7374.

Prescriptions and Renewals

You should request all prescriptions and authorizations for renewals during office hours.
